National view: each chain at its cheapest and its priciest province, both real prices, never averaged. Pick your province above for the exact figures where you live. Cash totals include each pharmacy's dispensing fee and assume no coverage. Rows marked Audited come from a published August 2026 retail phone audit; rows marked Estimated are modelled from list pricing and each chain's fee, so confirm those before relying on one. The savings-card column applies Ozempic savings card to that same total, and programmes generally cannot be combined with public plans such as ODB, NIHB or BC PharmaCare. Western-Canada-only chains appear in BC, AB, MB and SK.

Totals include the dispensing fee. Provinces without a Costco pharmacy (Quebec, New Brunswick, Prince Edward Island, Newfoundland and Labrador) lose the usual cheapest counter, which is most of the gap.

Or skip the pharmacy counter entirely

Licensed Canadian platforms will assess you and write a prescription without a family doctor. Some also dispense, quoting one monthly price that includes the medication, and the cheapest of those beats every pharmacy counter in Canada.

All-in providers include the medication in the monthly price, so compare those directly against the pharmacy totals above. Prescription-only platforms charge for the consult alone; you then pay a pharmacy separately, so add a row from the table above to get your real monthly cost. Fees are provider-reported and re-checked regularly.

Not yet dispensed in Canada
Health Canada approved Sevmia on 29 June 2026, but its Drug Product Database entry still shows no market date, so it is not yet dispensed by Canadian pharmacies. Supply depends on semaglutide active ingredient from Dr. Reddy's, and availability is not expected before November 2026. The figures below would be a guess, so we do not publish one.

Per-dose figures assume four doses per pen (one weekly injection). Rybelsus is a daily tablet, so its per-dose figure is per tablet; Saxenda is a daily injection.

City figures resolve to the cheapest counter in that market, including its dispensing fee. Chain coverage differs by city: Costco and the Western-Canada-only banners are not present everywhere, which is usually why a city ranks where it does. City notes are editorial and describe local markets; they are not price quotes.

Calgary, AB$248

Lowest big-city total we track. Costco Beacon Hill and three Superstore pharmacies compete inside a 6 km radius, and Alberta caps nothing; the market does it.

Edmonton, AB$249

Effectively tied with Calgary. Two Costco pharmacies plus dense Superstore coverage; south-side stores quote lower than downtown independents.

Red Deer, AB$252

Costco Gasoline Alley keeps this well below the national average for a city its size.

Lethbridge, AB$253

One Costco pharmacy; otherwise Superstore and Save-On set the floor.

Mississauga, ON$259

Cheaper than neighbouring Toronto on identical product. Four Costco pharmacies inside the city limits is the reason; worth the drive from west-end Toronto.

Brampton, ON$259

Two Costco pharmacies and heavy Superstore coverage make this one of the cheapest counters in the GTA.

Understanding city-level GLP-1 pricing

Why does the same GLP-1 pen cost different amounts in different Canadian cities?

The manufacturer’s price to the wholesaler is essentially uniform across Canada. What varies is everything downstream: each province sets its own rules for how pharmacists are paid, which produces very different dispensing lines on your receipt, and competition is local. A city with four Costco pharmacies inside its boundary has a price floor that a city with none simply does not have.

Which Canadian city is genuinely cheapest for GLP-1 medications?

Alberta cities lead consistently: no provincial cap forcing prices upward, and dense Costco and Real Canadian Superstore coverage in Calgary and Edmonton keeps the discount floor competitive. The city table above ranks every market we track in your province, and the note under it names the counter.

Is it worth driving to a cheaper suburb or neighbouring city?

Often, yes. The gap between a city and its own suburbs is frequently larger than the gap between provinces: Mississauga runs cheaper than Toronto proper because four Costco pharmacies operate inside Mississauga and none inside the City of Toronto. Before you drive, phone the specific pharmacy for the cash price of your exact strength including the dispensing fee, and confirm stock.

Do mail-order and online pharmacies beat local city pricing?

It depends on where you live. In large markets with strong discount-chain coverage a local Costco or Superstore counter usually still wins. In smaller and more remote markets, including much of Atlantic Canada, mail order is often the cheapest legitimate route because there is no local discounter to set a floor. Compare the all-in figure against the cheapest counter in your own city.

How are these prices checked?

Every row says. Rows marked Audited come from a published August 2026 retail phone audit of Canadian pharmacy counters. Rows marked Estimated are modelled from published list pricing and each chain’s dispensing fee, and should be confirmed with the pharmacy. A row marked Verified means we phoned that counter ourselves on the date shown. If a number here does not match what your pharmacy quotes, the pharmacy is right and we want to know.
